• Complete the New Account Form and Sales Tax Form
• Submit an opening order of at least $100.00 in retail value using the attached Order Form
• Either Fax the three completed forms to 1-800-214-1438

Once our customer service department has set you up with an account you will be free to place orders on our website 24 hours/day, 7 days/week. All you have to do is log in to your account by entering your Customer ID number and the zip code of your shipping address.

Note: New Customers must complete and submit the above forms before online ordering will be possible.

Absolutely! Once you have logged in, the website knows who you are and what discount, if any, applies to your orders.
